HOW TO REFER YOUR PATIENT TO SWEETGRASS


 
 

WE APPRECIATE ALL REFERRALS - yOUR PATIENTS ARE IMPORTANT TO US

  • Patients do not require a formal referral to be evaluated by our practice. Your notes and formal referral are always helpful and appreciated by us. The first step is to have your patient review the clinicians on this website to find a specialist that meets their needs and accepts their insurance. The second step is to advise your patient to personally fill out the online intake form in full (link listed below).

  • If you are an outpatient provider seeking to refer a patient for an appointment, please fax a referral or most recent clinical notes. We aim to contact your patient within less than 3 days of receiving your referral.

  • To receive clinical updates/patient intake note, please have your patient fill out a consent form to release information to you and include this in your fax.

  • If you are an inpatient provider seeking to refer a hospitalized patient, please fax your referral, a copy of the admission note, and a copy of the physician progress note, and we will contact you. We may require your patient to be present at the time of scheduling to ensure patient consent forms are completed.
